Mission, Vision, Goals and Values

The Mission of Empowered Living SLS is to empower individuals served to have meaningful, successful, and inclusive lives full of care and respect in the communities of their choice. We promote a person-centered approach that values every person’s privacy and dignity and respects their dreams while encouraging opportunities for joy.

The Vision of Empowered Living SLS: We visualize a world where individuals are empowered to choose the services and supports they want in order to be safe and achieve their dreams and aspirations.

The Goals for Empowered Living SLS are as follows:

Ensure individuals are safe in their homes and neighborhoods.
Empower individuals to live in a home of their choosing and to make decisions regarding their home life.
Support individuals in directing the person-centered services they receive, including the agencies who serve them.
Empower the person served to plan for their future including making informed decisions today that will set them up for success.
Ensure individuals are supported in their communication preferences, choices, and needs.
Encourage individuals to have relationships with family, friends and neighbors who will make up their circle of support for their own well-being.
Assist individuals in determining what kinds of community activities in which they may wish to participate and use local community and generic resources to help them join the clubs, groups, and churches they choose.
Collaborate with NBRC to understand and support the individual's program plan (IPP), provide support to individuals in their home and community and help the individuals achieve their ISP goals.

The Values for Empowered Living SLS are as follows:

Trustworthiness – we are committed to the people we serve. We build and maintain trust with them, their families, our employees, community members, and Regional Center representatives.
Nimble – we recognize that things are ever-changing in the lives of the people we support. It is important that we remain nimble and provide flexible and tailored support as changes occur.
Demand Respect and Celebrate Diversity – we respect each individual regardless of disability, ability, gender, race, nationality, creed, age, sexual orientation, gender identification/expression, culture, income level or social status.
Seek out Solutions – we seek to find solutions. We use breakthrough thinking to bring to life the dreams and aspirations of the individual.
Community Inclusiveness – the people we support will choose inclusive community opportunities and experience community membership.
Communication – we communicate in a timely, professional manner with all people while acknowledging that each person communicates differently and uses different modes of communication.
Culture – the team provides professional and proficient care to and are accountable for the success of the people they support. We are one team that embraces each other, builds each other up, and speaks up for the health & safety of our team and the people we support.
